From my quick research, the pretzel incident may have been a cardiac arrythmia, which could cause a stroke. This can be treated with a implanted cardiac defibrillator("ICD"). The LifeVest is an option for people who don't want to undergo surgery. Here are a couple relevant links:

I've been reviewing the end of the debate on my Tivo, the one place where you can get a good look at Bush's back, and there is definitely a U-shape object in his upper back that you can see as he crosses the screen to the right, at about 1:30:14 on the CSPAN footage.
